DONORS According to a recent independent audit, 93 cents of every dollar raised goes directly to patient care. The Neighborhood Health Clinic is entirely supported by non-governmental funding and its major sources of funding are individual donors and grants from civic organizations and church groups. The CEO, Ken Wilborn, is available to assist individuals and groups in the funding decisions and the Clinic provides energetic and inspirational speakers for groups that are interested in its mission. In additional to dollar donations, we encourage donations of medical equipment, medications, outside specialty services, food and food service and other Clinic necessities. Stock Transfer Instructions There are three easy ways to give securities to Neighborhood Health Clinic: 1. ELECTRONIC TRANSFER: IT IS VERY IMPORTANT THAT THE DONOR OR THEIR BROKER NOTIFY US (239-261-6600) OF THE IMPENDING TRANSFER. We must know the name of the stock, the number of shares, and the name of the donor. IF YOUR BROKER (OR BANKER) HOLDS THE STOCK IN STREET NAME: Instruct your broker in writing the specific shares that you wish to donate to Neighborhood Health Clinic. Have your broker call NHC to receive instructions on the disposition of the donated shares. Stock evaluation will be the date the transferred shares reach our account. For the reason it is important that your broker transfer stock in an expedient manner. The gift is complete only when the actual transfer takes place. 3. A DONOR BILL of RIGHTS PHILANTHROPY is based on voluntary action for the common good. It is a Tradition of giving and sharing that is primary to the quality of life. To have the opportunity for their names to be deleted from mailing lists that an organization may intend to share. 10. To feel free to ask questions when making. a donation and to receive prompt, truthful and forthright answers.
