According to a recent independent audit, 93 cents of every dollar raised goes directly to patient care.

The Neighborhood Health Clinic is entirely supported by non-governmental funding and its major sources of funding are individual donors and grants from civic organizations and church groups. The CEO, Ken Wilborn, is available to assist individuals and groups in the funding decisions and the Clinic provides energetic and inspirational speakers for groups that are interested in its mission. In additional to dollar donations, we encourage donations of medical equipment, medications, outside specialty services, food and food service and other Clinic necessities.

Stock Transfer Instructions


There are three easy ways to give securities to Neighborhood Health Clinic:

1. ELECTRONIC TRANSFER: IT IS VERY IMPORTANT THAT THE DONOR OR THEIR BROKER NOTIFY US (239-261-6600) OF THE IMPENDING TRANSFER.

We must know the name of the stock, the number of shares, and the name of the donor.

Stocks can be transferred electronically to Neighborhood Health Clinic’s account at Northern Trust with the following information:

Following IRS regulations, valuation of the gift will be based on the date the stock transfer into NHC’s account.

2. IF YOUR BROKER (OR BANKER) HOLDS THE STOCK IN STREET NAME:

Instruct your broker in writing the specific shares that you wish to donate to Neighborhood Health Clinic.

Have your broker call NHC to receive instructions on the disposition of the donated shares.

Stock evaluation will be the date the transferred shares reach our account. For the reason it is important that your broker transfer stock in an expedient manner. The gift is complete only when the actual transfer takes place.

3. IF YOU HOLD THE CERTIFICATES:

Send, UNENDORSED, certificate by registered mail to:

  • By SEPARATE first class mail, send a stock power signed exactly as your name appears on the UNENDORSED CERTIFICATE, with your signature guaranteed by a bank officer.
  • Stock powers can be obtained from your broker, banker. Include a letter indicating for what purpose the gift is being given. Stock valuation date will be the postmark date on the envelope containing the stock power.
  • If the certificate is made out in the name of Neighborhood Health Clinic, valuation will be made as of the issue date recorded on the certificate.

A DONOR BILL of RIGHTS

PHILANTHROPY is based on voluntary action for the common good. It is a Tradition of giving and sharing that is primary to the quality of life. To assure that philanthropy merits the respect and trust of the general public, and that donors and prospective donors can have full confidence in the not-for-profit organizations and causes they are asked to support, we declare that all donors have these rights:

1. To be informed of the organization’s mission, of the way the organization intends to use donated resources, and of its capacity to use donations effectively for their intended purposes.

2. To be informed of the identity of those serving on the organization’s governing board, and to expect the board to exercise prudent judgment in its stewardship responsibilities.

3. To have access to the organization’s most recent financial statements.

4. To be assured their gifts will be used for the purposes for which they were given.

5. To receive appropriate acknowledgment and recognition

6. To be assured that information about their donations is handled with respect and with confidentiality to the extent provided by law.

7. To expect that all relationships with individuals representing organizations of interest to the donor will be professional in nature.

8.To be informed whether those seeking donations are volunteers, employees of the organization or hired solicitors.

9. To have the opportunity for their names to be deleted from mailing lists that an organization may intend to share.

10. To feel free to ask questions when making. a donation and to receive prompt, truthful and forthright answers.
